The goal of the evaluation is to let the Special Worker know how they have been doing at their job, and facilitate a two-way conversation about how work has been going between the two of you; how effective communication has been, to set goals for the upcoming quarter and beyond, and ultimately how you both can communicate and work even better together, always keeping in mind the Fifth Tradition which states that MA’s primary purpose is “to carry its message to the marijuana addict who still suffers.”
Please email the Service Inventory Group at [email protected] with any questions or concerns regarding the review process or if you feel uncomfortable being honest in your feedback for any reason. This also includes concerns about the Special Worker you are reviewing, if you have concerns about the Special Worker seeing your evaluation answers, or during the review discussion meeting. Your email will remain confidential within the Service Inventory Group until SIG can address your concerns.
Scheduling Quarterly Review Meetings
Your Quarterly Review Meetings should be completed no later than 3 weeks before the end of the review month. Trustees, please schedule review meetings with the special worker(s) you work during the last two weeks of the quarterly review month.
Please email Service Inventory Group [email protected] when you’ve scheduled your review meeting with the Special Worker, and again after the review meeting happens to confirm that it occurred.
Emails sent to that address will be kept confidential until the person sending the email can be addressed to by the Service Inventory Group.

Don’t forget to refer to the materials in the Trustee Handbook about conducting an effective performance review, writing performance evaluations, and how to have a productive evaluation meeting. You can and should also consult with your fellow Trustees, if you’re seeking additional guidance and support beyond the written materials provided.
Instructions for this Special Worker Evaluation
There are a total of 23 questions and 5 categories. There are a total of 5 mandatory questions, please select at minimum 10 questions to answer, at least 2 in each category. Choose questions that best suit the current situation and what you feel needs to be discussed.
When this form has been submitted, a copy of your answers will be automatically emailed to you and saved to the Special Worker’s confidential/secure employee file. Additionally, a copy will be sent to Special Worker you are reviewing, and the Service Inventory Group.
The exception, is if you indicate in the first question, “NO that you DO NOT feel like you can provide a fair and unbiased evaluation,” all of the answers within this form will NOT be sent to the Special Worker you are reviewing. Instead, it will only be sent to the Service Inventory Group, who will intervene to address your concerns.
