These questions are intended to facilitate a two-way conversation between you and the trustee that you work with so that you can determine what’s working and what isn’t, set goals for the upcoming quarter/year, and review the progress and process of achieving these goals. The purpose of this review is not to critique you, as the special worker, but to determine how the Trustee and Special Worker can improve your team performance and working relationship as a team towards accomplishing the set of shared goals. Ideally, this type of communication is already happening regularly. This review process exists to provide an extra push to receive feedback and provide a forum to facilitate these conversations.
Instructions for this Self-Review
The goal of this process is to support your needs and to facilitate the type of review and discussion you want to have about your work with the Trustee(s) that supervise your work.
There are a total of 28 questions, including some with subsidiary questions. Aside from 4 mandatory questions, the remaining 24 questions are optional. Select any self-review questions that best suit your current situation and what you feel needs to be discussed then type your responses in the form.
When this self-review form has been submitted, a copy of your answers will be emailed to you and saved to your confidential/secure employee file. Additionally, a copy will be sent to the Trustee(s) who supervise your work, and the Service Inventory Group.
The exception, is if you indicate in the first question, “NO, you do not feel you will be able to receive a fair unbiased evaluation from the Trustee conducting your review,” in which case your self-review will only be sent to the Service Inventory Group, who will then intervene to speak with you about your concerns, and assist in ensuring you receive a fair review.

This also includes concerns about the Trustee who supervises your work, if you have concerns about the supervising Trustee seeing your self-review answers, issuing your performance review, or during the review discussion meeting. Your email will remain confidential within the Service Inventory Group until SIG can address your concerns.
